with your email address, the names and amounts of the
supplements and their expiration dates.
2. Package and send the supplements to
the following address (shipping costs are generally part of the
donation):

3. To receive supplements, make a donation
to Canine
Cancer Awareness for each desired item. Add $4.80 for shipping
costs for the first item, $1.00 per additional item. Mail your
desired supplements and check to:

everyone. The person donating the supplements will be helping another
dog family battle cancer. The person receiving the supplements
is reducing the cost of fighting cancer and extending their precious

both parties have donated to Canine Cancer Awareness will help
others who are battling this disease!
